Youth Connect Outreach Program is a one-to-one early intervention program designed to help young people (13-19) who need extra support. The goal of this program is to help youth stay or become connected to their family, community, and school and to strengthen connections to natural supports to prevent and/or reduce the risk of homelessness, isolation, and substance misuse.
Families, social workers, teachers, and counsellors can submit a referral via email Child, Youth & Family Connections to connect youth with a one-to-one Youth Connect worker.
